Police are appealing for help after a man went missing after checking out of a Victorian hospital two months ago.

The 38-year-old has not used his phone, accessed his bank account, or contacted any loved ones.

There are concerns for a father-of-three who has been missing for two months after checking himself out of hospital.

Adam, whose surname police did not disclose, was last seen walking on Bellarine St in Geelong at about 12.30am on April 27 after leaving Geelong University Hospital.

Police say they have carried out “extensive” searches across Geelong, Hoppers Crossing and Grovedale.

Water Police also searched the Barwon River earlier this week but found nothing.

Detective Senior Constable Travis Wright said police spoke to the 38-year-old near the river on the night he went missing, and that he was seen on the James Harrison Bridge.

He also has “associates” that sleep rough near the river.

Adam has not used his phone, accessed his bank account or contacted any family members or friends since he disappeared.

He does not have a car and will likely be on foot or using public transport.

There is no suggestion foul play is involved, Wright said.

“We just want him to be found safe and well.”

Adam’s cousin, Sally, said he has missed family celebrations, including his daughter’s first birthday and the birth of his sister’s first baby.

“It’s so out of character for him to just disappear without a trace,” she said.

“We want him to know that if he’s out there, we’re here and we care and just let us know that you’re okay and we miss you.”

Sally urged people to keep look for her cousin.

“We just want him home safe.”

“At the end of the day it doesn’t matter the circumstances ... we just want him home, that’s all we can ask for.”

CCTV of Adam leaving the hospital and walking away has been released by police.

In the footage, he was wearing a dark green jumper, dark grey tracksuit pants and black Nike shoes.

He is described as being around 180cm tall, bald with a brown beard, and has a neck tattoo with the words Jeremy Campbell.

Anyone who sees Adam is urged to call Geelong Police Station.
