Six-year-old girl dies after being discharged twice from Sydney Children’s Hospital with flu diagnosis

‘She was kind, had many friends and loved her family, friends and school.’

A little girl has died after being discharged from hospital twice in one day.

The six-year-old was first taken to the emergency department at Sydney Children’s Hospital in Randwick on Monday and discharged after a clinical assessment, the hospital said.

She returned a short time later, was diagnosed with influenza, and sent home again with at-home care instructions.

The girl was later admitted to Northern Beaches Hospital (NBH), where she died on Tuesday.

NSW Police confirmed the death and said a brief will be prepared for the coroner.

A spokesperson for NBH said the hospital was deeply saddened by the tragic loss.

“Our heartfelt condolences go out to the child’s family and loved ones during this incredibly difficult time,” they said.

NBH will undertake appropriate investigations with the Sydney Children’s Hospital Network (SCHN).

“Alongside Sydney Children’s Hospital Network, where the child received care earlier in the day, we will undertake the appropriate investigations in line with NSW Health policy, including a joint independent review.

“A coronial inquest has also been called into the child’s passing.”

A SCHN spokesperson also confirmed a joint independent clinical governance process will take place.

“SCHN expresses our deepest condolences to the family for the loss of their child,” the spokesperson said.

“SCHN is progressing an initial review into the patient’s care while at the hospital.

“Based on this initial review, SCHN will work with Northern Sydney Local Health District as part of the joint independent clinical governance process.

“SCHN understands the matter has also been referred to the Coroner.

“Due to patient privacy, no further information can be provided at this time.”

NSW Premier Chris Minns said a full investigation is clearly needed.

“I’m obviously shocked by it, and I just feel terrible for the family involved,” he said.

“To not have that baby return home would be devastating for that family.

“There clearly needs to be a full investigation.

“We want to make sure no other family is in this kind of a situation.”

NSW Health Minister Ryan Park said he was heartbroken to learn of the girl’s death and mindful of how distressing such events are for hospital staff.

“I wish to express my deepest condolences to the family for their devastating loss,” Park said.

“I’m also mindful of how distressing these events are for staff, and I know they’ll be impacted by this tragedy.

“I want to understand how and why this tragedy has occurred and the parents deserve answers.

“The matter has now been referred to the Coroner and is also the subject of a joint Serious Adverse Event Review.”

The girl, a student at St Luke’s Grammar School in Dee Why, has been remembered as a gorgeous Kindergarten student full of life and energy.

“She was kind, had many friends and loved her family, friends and school,” principal Geoff Lancaster said in a statement.

“She began her journey at St Luke’s in Cottage last year and quickly captured the hearts of those around her with her warmth, vivacity and eagerness to learn.”

One of her teachers described her as “a thoughtful and caring student with a kind nature, who enjoyed play and many close friendships in Kindergarten”.

The Head of Junior School called her “bright, vivacious, kind and free spirited”.

“She was so full of light and joy. She was adored by her parents, teachers and the many friends she made at St Luke’s,” the statement read.

The tight-knit school community has been shocked by the unexpected loss, and a memorial service is being planned to honour the girl’s life.

The school is providing support to the family, friends, staff and students during this difficult time.
